PATHWAYS helps patients and families throughout all phases of treatment from aggressive to palliative care. Living with advanced or chronic illness is challenging. PATHWAYS helps address these challenges.

The PATHWAYS facilitator works with your physician to offer education on topics such as:

• Communications with family or medical care providers
• Defining quality of life
• Coping strategies
• Practical concerns and networking of appropriate resources
• Emotional pain or anxiety
• Spiritual needs
• Advanced directives

The PATHWAYS may be beneficial to patients who:

• Have a chronic or life-threatening disease
• Experience frequent hospitalizations or visits to the emergency room
• Need education on important medical decisions or treatments
• Recognize treatments that were previously beneficial are no longer working

Depending on your preference, consultations can be provided in the comfort of a home, physician office, hospital, or another agreed-upon location.  It is important to know that this program does not involve medical care, which continues to be directed by the patient's physician.

PATHWAYS services and consultations are offered at no cost to patients and families.

PATHWAYS is a community outreach service of The Hospice of East Texas in partnership with Texas Palliative Care.
