How To Request Patient Medical Records

To request a patient’s medical records, written consent must be obtained from the patient or the patient’s personal representative. A personal representative is an individual legally authorized to make health care decisions on the patient’s behalf. The Hospice of East Texas will respond to such requests; however, the following guidelines apply:

  • Consent must be in writing.
  • The patient’s consent must be voluntary and informed.
  • The written consent must be signed and datedd by the patient, guardian or personal representative.

Any consent to release clinical records must clearly state that The Hospice of East Texas is authorized to release the information and must include:

  • The patient’s name.
  • The specific information to be disclosed.
  • The purpose for which the information is being released.
  • The identity of the individual requesting the records, as well as their leagal authority to accesss the records will be verified.
